REACH Community Connector


Job description

As a REACH Community Connector, you will help participants access local activities, services and peer support identified in their agreed action plans. REACH is a project supported by PEACEPLUS; a programme managed by the Special EU Programmes Body (SEUPB). You will build strong community links, maintain an up-to-date resource map and electronic menu, reduce barriers to participation, support cross-border peer activity and record follow-up and outcomes accurately. If you are organised, approachable and confident working with people and community partners, we would love to hear from you. The role focuses on community navigation and engagement: developing and maintaining a comprehensive map and electronic menu of community resources, brokering relationships with voluntary, community, statutory and public sector partners, and reducing practical, social and confidence-related barriers to participation. The post holder will work across NI and ROI jurisdictions as required, supporting cross-border peer support groups and community engagement, maintaining accurate participant records on the Management Information System, and providing appropriate follow-up and onward signposting in line with agreed action plans, data protection and confidentiality requirements.

Responsibilities

  • Support participants to access appropriate community activities, groups and services in line with needs and action plans agreed with Social Prescribing Officers.

Skills and Qualifications

Essential Skills

  • Maintain accurate records.
  • Experience of helping people access community activities, services or support.
  • Person-centred approach that promotes choice and independence.
  • Communication, networking and organisational skills.

Desirable Skills

  • A relevant qualification and experience of community partnerships, group or peer support, outreach, resource mapping or cross-border working would be desirable.
